Glutamate
Glutamate is the most important transmitter for normal brain function. Nearly all excitatory neurons in the CNS are glutamatergic, and it is estimated that more than half of all brain synapses release this neurotransmitter. During brain trauma, there is excessive release of glutamate that can produce excitotoxic brain damage.

Glutamate is a nonessential amino acid that does not cross the blood-brain barrier and therefore must be synthesized in neurons from local precursors. The most prevalent precursor for glutamate synthesis is glutamine, which is taken up into presynaptic terminals by the system A transporter 2 (SAT2) and is then metabolized to glutamate by the mitochondrial enzyme glutaminase (Figure 6.5). Glucose metabolized by neurons also can be used to synthesize glutamate by transamination of 2-oxoglutarate, an intermediate of the tricarboxylic acid (Krebs) cycle. Glutamate synthesized in the presynaptic cytoplasm is packaged into synaptic vesicles by vesicular glutamate transporters (VGLUTs). At least three different VGLUT genes have been identified, with different VGLUTs involved in packaging glutamate into vesicles at different types of glutamatergic presynaptic terminals.

Once released, glutamate is removed from the synaptic cleft by the excitatory amino acid transporters (EAATs). EAATs are a family of five different Na+-dependent glutamate co-transporters. Some EAATs are present in glial cells and others in presynaptic terminals. Glutamate transported into glial cells via EAATs is converted into glutamine by the enzyme glutamine synthetase. Glutamine is then transported out of the glial cells by a different transporter, the system N transporter 1 (SN1), and transported into nerve terminals via SAT2. This overall sequence of events is referred to as the glutamate–glutamine cycle (see Figure 6.5). This cycle allows glial cells and presynaptic terminals to cooperate both to maintain an adequate supply of glutamate for synaptic transmission and to rapidly terminate postsynaptic glutamate action.

There are several types of ionotropic glutamate receptors (see Figure 6.3F). AMPA receptors, NMDA receptors, and kainate receptors are named after the agonists that activate them: AMPA (a-amino-3-hydroxyl-5-methyl-4-isoxazole-propionate), NMDA (N-methyl-d-aspartate), and kainic acid. All of these receptors are glutamate-gated cation channels that allow the passage of Na+ and K+, similar to the nAChR. Hence AMPA, kainate, and NMDA receptor activation always produces excitatory postsynaptic responses.

Most central excitatory synapses possess both AMPA and NMDA receptors. Antagonist drugs that selectively block either AMPA or NMDA receptors are often used to identify synaptic responses mediated by each receptor type. Such experiments reveal that excitatory postsynaptic currents (EPSCs) produced by NMDA receptors are slower and last longer than those produced by AMPA receptors (Figure 6.6A). EPSCs generated by AMPA receptors usually are much larger than those produced by other types of ionotropic glutamate receptors, so that AMPA receptors are the primary mediators of excitatory transmission in the brain. The physiological roles of kainate receptors are less well defined; in some cases, these receptors are found on presynaptic terminals and serve as a feedback mechanism regulate glutamate release. When found on postsynaptic cells, kainate receptors generate EPSCs that rise quickly but decay more slowly than those mediated by AMPA receptors (Figure 6.6B).

Like all ionotropic receptors, AMPA receptors are composed of multiple subunits. There are four different AMPA receptor subunits, designated GluA1 to GluA4 (see Figure 6.3F), with each subunit conferring unique functional properties to AMPA receptors. AMPA receptor subunits have several different domains, including an extracellular ligand-binding domain that is responsible for binding glutamate, and a transmembrane domain that forms part of the ion channel (Figure 6.7A). These subunits are organized into the tetrameric structure shown in Figure 6.7B– The extracellular structure of AMPA receptors, unlike that of nAChRs, is asymmetrical and therefore looks different when viewed from its front and side surfaces (see Figure 6.7B,C). The AMPA receptor is Y-shaped (see Figure 6.7B), with the large extracellular domains of the subunits narrowing down as the receptor passes through the plasma membrane (see Figure 6.7D). The extracellular ligand-binding domains have a characteristic “clamshell” shape, with glutamate and other ligands binding within the opening of the clamshell (circle in Figure 6.7C). The transmembrane domain consists of helices that form both the channel pore and a gate that occludes the pore when glutamate is not bound to the receptor. Binding of glutamate causes the clamshell structure to “shut”; this movement then causes the gate helices within the transmembrane domain to move and thereby open the channel pore (Figure 6.7E).

NMDA receptors have physiological properties that set them apart from the other ionotropic glutamate receptors. Perhaps most significant is that the pore of the NMDA receptor channel allows the entry of Ca2+ in addition to Na+ and K+. As a result, excitatory postsynaptic potentials (EPSPs) produced by NMDA receptors increase the concentration of Ca2+ in the postsynaptic neuron, with Ca2+ then acting as a second messenger to activate intracellular signaling processes (see Chapter 7). Another key property is that Mg2+ blocks the pore of this channel at hyperpolarized membrane potentials, while depolarization pushes Mg2+ out of the pore (Figure 6.8A). This imparts a peculiar voltage dependence to current flow through the receptor (Figure 6.8B, red line); removing extracellular Mg2+ eliminates this behavior (blue line), which demonstrates that Mg2+ confers the voltage dependence. Because of this property, NMDA receptors pass cations (most notably Ca2+) only when the postsynaptic membrane potential is depolarized, such as during activation of strong excitatory inputs and/or during action potential firing in the postsynaptic cell. This requirement for the coincident presence of both glutamate and postsynaptic depolarization to open NMDA receptors is widely thought to underlie some forms of synaptic information storage, such as longterm synaptic plasticity (see Chapter 8). Another unusual feature of NMDA receptors is that their gating requires a co-agonist—the amino acid glycine, which is present in the ambient extracellular environment of the brain.

NMDA receptors are tetrameric assemblies of subunits with many similarities to AMPA receptors. There are three groups of NMDA receptor subunits (GluN1, GluN2, and GluN3), with a total of seven different types of subunits (see Figure 6.3F). While GluN2 subunits bind glutamate, GluN1 and GluN3 subunits bind glycine. NMDA receptor tetramers typically comprise two glutamate-binding subunits (GluN2) and two glycine-binding subunits (GluN1). In some cases, GluN3 replaces one of the two GluN2 subunits. This mix of subunits ensures that the receptor binds both to glutamate released from presynaptic terminals and to the ambient glycine co-agonist.

The structure of an NMDA receptor resembles a hot-air balloon (Figure 6.8C,D). Similar to AMPA receptor subunits, NMDA receptor subunits possess clamshell-shaped ligand-binding domains that bind to glutamate and to glycine, as well as transmembrane domains that form the channel pore and gate. One unique feature of NMDA receptors is a structure in the extracellular vestibule, adjacent to the transmembrane domain, that is postulated to bind Ca2+ and may help confer Ca2+ permeability to NMDA receptors (Figure 6.8E). Gating of NMDA receptors is proposed to arise from closure of the ligand-binding domains upon binding of glutamate and glycine, leading to a conformational change that opens the channel pore; in contrast, binding of antagonists to the ligand-binding domains displaces them and prevents channel opening (Figure 6.8F). The site at which Mg2+ binds to block the pore of the NMRA receptor has not yet been identified.

In addition to these ionotropic glutamate receptors, there are three classes of metabotropic glutamate receptors (mGluRs; see Figure 6.4C). These receptors differ in their coupling to intracellular signal transduction pathways (see Chapter 7) and in their sensitivity to pharmacological agents. Activation of many of these receptors leads to inhibition of postsynaptic Ca2+ and Na+ channels. Unlike the excitatory ionotropic glutamate receptors, mGluRs cause slower postsynaptic responses that can either excite or inhibit postsynaptic cells. As a result, the physiological roles of mGluRs are quite varied. Although they possess a transmembrane domain that spans the membrane seven times, characteristic of all G-protein-coupled receptors, mGluRs are structurally unique because they are dimers of two identical subunits (Figure 6.9). Each subunit possesses a venus flytrap domain, a glutamate-binding domain similar to the clamshell-shaped ligand-binding domains of ionotropic glutamate receptors (see Figures 6.7 and 6.8). This venus flytrap domain is connected to the transmembrane domain via a linker domain rich in the amino acid cysteine (see Figure 6.9, left). Binding of glutamate causes the venus flytrap domains to close, with the resulting movement causing the transmembrane domains to rotate and thereby activate the receptor (see Figure 6.9, right). Binding of G-proteins to the activated receptor then initiates intracellular signaling.
