Anesthesia and Neuronal Electrical Signaling
Anesthesia is an essential part of clinical practice and refers to procedures that reduce sensation during surgical procedures, most often to alleviate pain (see Chapter 10) or to create a state of unconsciousness. The drugs that produce anesthesia are called anesthetics, and these agents usually work by interfering with the electrical signaling mechanisms of neurons. There are three broad categories of anesthesia.

The mildest form of anesthesia, local anesthesia, is used to prevent pain sensation in localized parts of the body. Local anesthetics, such as procaine and lidocaine, block action potential propagation along peripheral nerves (Figure A) by blocking the Na+ channels involved in action potential generation (see Chapter 4). This causes a loss of sensory perception commonly called numbing. Local anesthesia can be highly restricted in its range of action, due to topical application of local anesthetics onto (or injection into) the target tissue. Perhaps the most widespread example of local anesthesia is the injection of procaine into the mandible to block action potential conduction in the inferior alveolar nerve, thereby preventing pain sensation in a portion of the mouth during dental procedures.

Regional anesthesia desensitizes a larger region of the body and is typically produced by injecting local anesthetics near the spinal cord to prevent pain sensation in the region where a surgical procedure will be performed. For example, more than 50% of women giving birth in U.S. hospitals have local anesthetics injected into the epidural space of their spinal canal to prevent pelvic pain associated with labor. Although patients receiving regional anesthesia remain conscious, they sometimes additionally receive a sedative agent (also called a tranquilizer) to reduce anxiety or induce sleep. Many sedatives enhance the activity of the postsynaptic GABA receptors found at most inhibitory synapses (see Chapter 6) and the resulting strengthening of synaptic inhibition reduces neuronal activity and presumably causes sedation (Figure B) (see Chapter 5). One class of sedative is the barbiturates, which include pentobarbital. Because of their addictive properties, as well as their lethality at high doses, barbiturates have largely been replaced by benzodiazepines. The best-known benzodiazepine is diazepam, more commonly known as valium. A related compound is zolpidem (Ambien), an imidazopyridine sedative that is also used as a hypnotic agent to induce sleep. Propofol is yet another type of sedative that works primarily by enhancing GABA receptor activity, though it also blocks Na+ channels in a way similar to that of local anesthetics. An overdose of propofol and benzodiazepines was responsible for the death of the popular musician Michael Jackson.

General anesthesia causes unconsciousness, an absence of sensation and muscular relaxation. It is used for major surgical procedures, such as those of long duration and/or involving substantial loss of blood. General anesthetic agents can be injected into the circulatory system (intravenous anesthetics, Figure C) or inhaled through the respiratory system (inhalation anesthetics, Figure D).

General anesthesia can be induced by injection of higher doses of some of the sedative agents described above that work on synaptic GABA receptors. Because these agents do not prevent pain sensation, they are often administered along with analgesic agents, such as fentanyl, that alleviate pain by acting on the receptors for opiate peptides (see Chapter 6) of peripheral and central pain pathways (see Chapter 10). Longer-acting opiate receptor drugs are used for postoperative analgesia; the best-known example of such drugs is morphine. Another intravenous anesthetic is ketamine, a drug that blocks the NMDA-type glutamate receptors found at many excitatory synapses (Figure C) see Chapter 6) but that also works on GABA and opiate receptors. Presumably the resulting reduction in excitatory synaptic transmission underlies the anesthetic actions of ketamine. Use of ketamine is limited by its diverse side effects, including production of hallucinations.

Inhalation anesthetics are volatile liquids that vaporize at room temperature and can then be inhaled. The first and best-known example of an inhalant anesthetic is diethyl ether, which is no longer used clinically because it is highly flammable. More widely used inhalant anesthetics include halothane and isoflurane. Although the mechanism of action of inhalation anesthetics is not established, one leading hypothesis is that they hyperpolarize the resting membrane potential of neurons and thereby make it more difficult to fire action potentials (Figure D). This action is thought to be caused by the anesthetics opening the 2-P K+ channels that create the resting membrane potential (see Chapter 4).

In conclusion, understanding the mechanisms underlying anesthetic action both illuminates how these clinically valuable agents work and emphasizes the fundamental importance of neuronal electrical signaling for the function of the nervous system.

How Ion Movements Produce Electrical Signals
Electrical potentials are generated across the membranes of neurons—and, indeed, across the membranes of all cells— because (1) there are differences in the concentrations of specific ions across nerve cell membranes, and (2) these membranes are selectively permeable to some of these ions. These two conditions depend in turn on two different kinds of proteins in the plasma membrane (Figure 2.4). The ion concentration gradients are established by proteins known as active transporters, which, as their name suggests, actively move ions into or out of cells against their concentration gradients. The selective permeability of membranes is due largely to ion channels, proteins that allow only certain kinds of ions to cross the membrane in the direction of their concentration gradients. Thus, channels and transporters basically work against each other, and in so doing they generate the resting membrane potential, action potentials, and the synaptic potentials and receptor potentials that trigger action potentials. Chapter 4 describes the structure and function of these channels and transporters.

To appreciate the role of ion gradients and selective permeability in generating a membrane potential, consider a simple system in which an artificial membrane separates two compartments containing solutions of ions. For comparison to the situation in neurons, we will refer to the left compartment as the inside and call the right compartment the outside. In such a system, it is possible to control the composition of the two solutions and thereby control the ion gradients across the membrane. It is also possible to control the ion permeability of the membrane.

As a first example, consider the case of a membrane that is permeable only to potassium ions (K+). If the concentration of K+ on each side of this membrane is equal, then no electrical potential will be measured across it (Figure 2.5A). However, if the concentration of K+ is not the same on the two sides, then an electrical potential will be generated. For instance, if the concentration of K+ in the inside compartment is ten times higher than the K+ concentration in the outside compartment, then the electrical potential of the inside will be negative relative to that of the outside (Figure 2.5B). This difference in electrical potential is generated because the potassium ions flow down their concentration gradient and take their electrical charge (one positive charge per ion) with them as they go. Because neuronal membranes contain pumps that accumulate K+ in the cell cytoplasm, and because potassium-permeable channels in the plasma membrane allow a transmembrane flow of K+, an analogous situation exists in living nerve cells. As will be proved later in the chapter, such an efflux of K+ is responsible for the resting membrane potential.

In the hypothetical case just described, an equilibrium will quickly be reached. As K+ moves from the inside compartment to the outside (the initial conditions; shown on the left in Figure 2.5B), a potential is generated that tends to impede further flow of K+. This impediment results from the fact that the potential gradient across the membrane tends to repel the positive potassium ions that would oth-erwise move across the membrane. Thus, as the outside becomes positive relative to the inside, the increasing positivity makes the outside less attractive to the positively charged K+. The net movement (or flux) of K+ will stop at the point (“At equilibrium” in the right panel of Figure 2.5B) where the potential change across the membrane (the relative positivity of the outside compartment) exactly offsets the concentration gradient (the tenfold excess of K+ in the inside compartment). At this electrochemical equilibrium, there is an exact balance between two opposing forces (see Figure 2.5B): (1) the concentration gradient that causes K+ to move from inside to outside, taking along positive charge, and (2) an opposing electrical gradient that increasingly tends to stop K+ from moving across the membrane. The number of ions that needs to flow to generate this electrical potential is very small (approximately 10–12 moles of K+ per cm2 of membrane, or less than one millionth of the K+ ions present on each side). This last fact is significant in two ways. First, it means that the concentrations of permeant ions on each side of the membrane remain essentially constant, even after the flow of ions has generated the potential. Second, the tiny fluxes of ions required to establish the membrane potential do not disrupt chemical electroneutrality because each ion has an oppositely charged counter-ion (chloride ions in Figure 2.5) to maintain the neutrality of the solutions on each side of the membrane. The concentration of K+ remains equal to the concentration of Cl– in the solutions in both compartments, meaning that the separation of charge that creates the potential difference is restricted to the immediate vicinity of the membrane.
