Unconventional Neurotransmitters

In addition to the conventional neurotransmitters already described, some unusual molecules are used for signal- ing between neurons and their targets. These chemical signals can be considered as neurotransmitters because of their roles in interneuronal signaling and because their release from neurons is regulated by Ca2+. How- ever, they are unconventional in comparison with other neurotransmitters because they are not stored in synaptic vesicles and are not released from presynaptic terminals via exocytotic mechanisms. In fact, these unconventional neurotransmitters need not be released from presynaptic terminals at all and are often associated with retrograde signaling (that is, from postsynaptic cells back to presyn- aptic terminals).

Endocannabinoids are a family of related endogenous signals that interact with cannabinoid receptors. These re- ceptors are the molecular targets of D9-tetrahydrocannab- inol, the psychoactive component of the marijuana plant, Cannabis (Box 6C). While some members of this emerg- ing group of chemical signals remain to be determined, anandamide and 2-arachidonoylglycerol (2-AG) have been established as endocannabinoids. These signals are unsat- urated fatty acids with polar head groups and are produced by enzymatic degradation of membrane lipids (Figure 6.23A,B). Production of endocannabinoids is stimulated by a second messenger within postsynaptic neurons, typically a rise in postsynaptic Ca2+ concentration, allowing these hydrophobic signals to diffuse through the postsynaptic membrane to reach cannabinoid receptors on other nearby cells. Endocannabinoid action is terminated by carrier-me- diated transport of these signals back into the postsynaptic neuron, where they are hydrolyzed by the enzyme fatty acid hydrolase (FAAH).

At least two types of cannabinoid receptors have been identified, with most actions of endocannabinoids in the CNS mediated by the CB1 type (see Box 6C). The CB1 re- ceptor is a G-protein-coupled receptor related to the me- tabotropic receptors for ACh, glutamate, and other con- ventional neurotransmitters. Several compounds that are structurally related to endocannabinoids and that bind to the CB1 receptor have been synthesized (Figure 6.23C). These compounds act as agonists or antagonists of the CB1 receptor and serve both as tools for elucidating the physi- ological functions of endocannabinoids and as targets for developing therapeutically useful drugs.

Medicinal use of the marijuana plant (Cannabis sativa) (Fig- ure  A)  dates  back  thousands of years. Ancient civilizations—includ- ing both Greek and Roman societies in Europe, as well as Indian and Chinese cultures in Asia—appreciated that this plant was capable of producing relax- ation, euphoria, and several other psy- chopharmacological actions. In more recent times, medicinal use of marijua- na has revived and the recreational use of marijuana has become so popular that some societies have decriminalized its use.

Understanding the brain mecha- nisms underlying the actions of marijua- na was advanced by the discovery that a cannabinoid, D9-tetrahydrocannabi- nol (THC; Figure B), is the active compo- nent of marijuana. This finding led to the development of synthetic derivatives, such as WIN 55,212-2 and rimonabant (see Figure 6.23), that have served as valuable tools for probing the brain ac- tions of THC. Of particular interest is that receptors for these cannabinoids exist in the brain. The best studied of these re- ceptors, called CB1, is a metabotropic receptor that activates G-protein sig- naling pathways (Figure C). CB1 exhibits marked regional variations in distribu- tion, being especially enriched in brain areas—such as the substantia nigra and caudate putamen—that have been im- plicated in drug abuse (Figure D). The presence of these brain receptors for cannabinoids led in turn to a search for endogenous cannabinoid compounds in the brain, culminating in the discovery of endocannabinoids such as 2-AG and anandamide (see Figure 6.23). This path of discovery closely parallels the identifi- cation of endogenous opioid peptides, which resulted from the search for en- dogenous morphine-like compounds in the brain (see text and Table 6.2).

Given that THC interacts with brain endocannabinoid receptors, particular- ly the CB1 receptor, it is likely that such actions are responsible for the behav- ioral consequences of marijuana use. Indeed, many of the well-documented effects of marijuana are consistent with the distribution and actions of brain CB1 receptors. For example, marijuana’s ef- fects on perception could be due to CB1 receptors in the neocortex, effects on psychomotor control due to endocan- nabinoid receptors in the basal ganglia and cerebellum, effects on short-term memory due to cannabinoid recep- tors in the hippocampus, and the well- known effects on stimulating appetite due to hypothalamic actions. While formal links between these behavioral con- sequences of marijuana and the under- lying brain mechanisms are still being forged, studies of the actions of this drug have shed substantial light on basic syn- aptic mechanisms, which promise to fur- ther elucidate the mode of action of one of the world’s most popular drugs.

Endocannabinoids participate in several forms of synap- tic regulation. The best-documented action of these agents is the inhibition of communication between presynaptic inputs and their postsynaptic target cells. In both the hip- pocampus and the cerebellum (among other brain regions), endocannabinoids serve as retrograde signals that regulate GABA release at certain inhibitory synapses. At such syn- apses, depolarization of the postsynaptic neuron causes a transient reduction in inhibitory postsynaptic responses (Figure 6.24). Depolarization reduces synaptic transmission by elevating the concentration of Ca2+ in the postsynaptic neuron; this rise in Ca2+ triggers synthesis and release of endocannabinoids from the postsynaptic cells. The endo- cannabinoids then bind to CB1 receptors on presynaptic ter- minals, inhibiting the amount of GABA released in response to presynaptic action potentials, and thereby reducing in- hibitory transmission. The mechanisms responsible for the reduction in GABA release are not entirely clear but proba- bly involve effects on voltage-gated Ca2+ channels and/or K+ channels in the presynaptic neurons.

Nitric oxide (NO) is an unusual and especially inter- esting chemical signal. It is a gas produced by the action of nitric oxide synthase, an enzyme that converts the amino acid arginine into a metabolite (citrulline) and simultane ously generates NO (Figure 6.25). Within neurons, NO synthase is regulated by Ca2+ binding to the Ca2+ sensor protein calmodulin (see Chapter 7). Once produced, NO can permeate the plasma membrane, meaning that NO generated inside one cell can travel through the extracellu- lar medium and act inside nearby cells. Thus, this gaseous signal has a range of influence that extends well beyond the cell of origin, diffusing a few tens of micrometers from its site of production before it is degraded. This property makes NO a potentially useful agent for coordinating the activities of multiple cells in a localized region and may mediate certain forms of synaptic plasticity that spread within small networks of neurons.

All of the known actions of NO are mediated within its cellular targets; for this reason, NO often is considered a second messenger rather than a neurotransmitter. Some of the actions of NO are due to the activation of the enzyme guanylyl cyclase, which then produces the second messenger cGMP within target cells (see Chapter 7). Other actions of NO are the result of covalent modification of target proteins via nitrosylation, the addition of a nitryl group to selected amino acids within the proteins. NO decays spontaneously by reacting with oxygen to produce inactive nitrogen oxides; thus, its signals last for only a short time (seconds or less). NO signaling evidently regulates a vari- ety of synapses that also employ conventional neurotrans- mitters; so far, presynaptic terminals that release glutamate are the best studied NO targets in the CNS. NO may also be involved in some neurological diseases. For example, it has been proposed that an imbalance between nitric oxide and superoxide generation underlies some neurodegener- ative diseases.

Summary

The complex synaptic computations occurring at neural circuits throughout the brain arise from the actions of a large number of neurotransmitters, which act on an even larger number of postsynaptic neurotransmitter receptors. Glutamate is the major excitatory neurotransmitter in the brain, whereas GABA and glycine are the major inhibitory neurotransmitters. The actions of these small-molecule neurotransmitters are typically faster than those of the neuropeptides. Thus, most small-molecule transmitters mediate synaptic transmission when a rapid response is essential, whereas the neuropeptide transmitters, as well as the biogenic amines and some small-molecule neu- rotransmitters, tend to modulate ongoing activity in the brain or in peripheral target tissues in a more gradual and ongoing way. Two broadly different families of neurotrans- mitter receptors have evolved to carry out the postsynaptic signaling actions of neurotransmitters. Ionotropic or li- gand-gated ion channels combine the neurotransmitter re- ceptor and ion channel in one molecular entity, and there- fore give rise to rapid postsynaptic electrical responses. Metabotropic receptors regulate the activity of postsyn- aptic ion channels indirectly, usually via G-proteins, and induce slower and longer-lasting electrical responses. Me- tabotropic receptors are especially important in regulating behavior, and drugs targeting these receptors have been clinically valuable in treating a wide range of behavioral disorders. The postsynaptic response at a given synapse is determined by the combination of receptor subtypes, G-protein subtypes, and ion channels that are expressed in the postsynaptic cell. Because each of these features can vary both within and among neurons, a tremendous diver- sity of transmitter-mediated effects is possible. Drugs that influence transmitter actions have enormous importance in the treatment of neurological and psychiatric disorders, as well as in a broad spectrum of other medical problems.
